That sounds like 2 drastically different approaches to treating heartworm. The first sounds like he was proposing treating the worms and heartworm treatment after they have the disease IS very expensive.
The other approach sounds like he was just using the regular monthly preventative medication to treat heartworms which is not the way to go and can be dangerous to give if they actually have a "developed" case of heartworm. Hmmmmmm
I would go with the first in this "mystery case" with Toby.

Well, It was just the larva.. And since heartworm pills kill them.. It worked out well and the dog is heartworm free now. :)

Toby's home!! They released him on their own without me having to make a big fuss about the whole thing. I don't think they knew what it was so they charged me $50 for a 2 night stay and for 2 injections. I can't complain since I thought it was going to be a lot more than that! He looks like he lost a little weight but will gain it back quickly.. And I noticed he has forming balls now!

He was SO excited to get out of there he was squirming and yelping/howling the whole way home.. When we got home he romped around the yard then peed and we came inside. First thing he got a bath, Omg.. He stunk SO bad but Piper was even worse. After that I let him see Krillon and he was oh so happy.. They played and played and are still at it. Heheh. I'm SO happy he's home and specially for Thanksgiving.

I was really worried that the Vet wasn't telling you anything, and also when you posted that he had feces matted in his fur - it didn't sound like they were giving him very good care at all.

When you mention you can see his 'balls forming' - couldn't you see them before? If Toby's having trouble with his testicles not staying descended (I've known a pup where one was down some days, and "hidden" on others) that could certainly be causing some internal upset.

Nope.. He never had balls before.. Well, From Thursday-Monday. I guess they are starting to drop. He's only 7 weeks old. :)
