Well, you can't. Or at least, such a chart would be pretty meaningless. Just like human babies, the size and weight of puppies varies significantly both at birth and as they grow. And that can be with puppies from the same litter, let alone unrelated pups.
If you take a look back through the (incredibly) numerous previous threads about puppy weight, you'll see that there is a very wide range. Very few of them give a size indication though, without which a pup's weight is pretty meaningless. What really matters is that each puppy is a healthy weight for it's size - and the best way of judging that is by how a pup looks (whether it's fat or thin) and how it behaves (ie. alert and playful) rather than trying to compare its weight with others that may be a very different size at a similar age.
