K9 Idiopathic Hepatitis anyone?

I took Tenaia to the vet Friday suspecting her thyroid was out of balance (she has been on Soloxine for several years now) as she has been very flakey, oily, and her coat had dulled in appearance. She also has balance issues, especially in her hind end, which we have known for a while but has previously been attributed to severe arthritis, and she takes Rimadyl to help with the pain and stiffness. Recently, she has experienced some incontinence while she is sleeping & I had chalked that up to the fact she is nearing 13 and sleeps so soundly she just doesn't bother to get up ~ in hindsight, probably should not have disounted that issue. Additionally, she now paces relentlessly and pants more than she normally did.

Turns out her thyroid is under control, but her liver and kidney enzymes are both really high ~ the big concern is her liver, as it has shot up considerably since she had surgery in April to remove some benign lumps that had grown to the point the vet thought one might burst if not removed within 6 months.

She went back on Monday to have a urinalysis to rule out any kidney or UT infections and also had abdominal xrays to see if there were any tumors present. Since neither was the case, we scheduled an appointment with the radiologist yesterday to have an ultrasound and liver biopsy to determine exactly what is going on and how to proceed with treatment. The suspicion is K9 idiopathic hepatitis (I will also be asking about Cushings when he calls with the results, as it seems after searching these forums, Cushings is pretty common amoung you all, and I have yet to find a thread relating to hepatitis). Lepto of course was also a fit to the symptoms, but we have absolutely no risk as Tenaia is homebound or on a leash if we are out walking because she is deathly allergic to all vaccines (including rabies) so we just don't take her anywhere, and we use a lix-it so we have no standing water at all - not even a dish.

While I realize she is almost 13, the paperwork didn't sound terribly positive, though she has no jaundice so hopefully this is in a very early stage. Has anyone had any experience with this disease in a dog? It is apparently nothing like the human versions of hep.

Sorry this is so long, but you all know how hard peace of mind is to come by, and there is nothing better than some good boxerworld support.

Update on Tenaia

Histopath/biopsy came back positive for hepatitis, so at least we know what is wrong. Thankfully I did not just assume her thyroid was a little off and take a wait & see approach to her issues as it seems we have caught this in an early stage.

The good news is there is no evidence of cancer, her heart and kidneys are fine, and despite the hepatitis, which causes fibrosis and basically destroys the liver, she still has a large amount of healthy liver cells. The bad news: hepatitis is not curable, and things they do for humans are not done for dogs. She is however, in the "treatable" stage (thank God!) since it was caught early, otherwise we would only be able to "manage" for comfort until the liver fails. Unfortunately, it means steroids for the rest of her life, however long we may be blessed to have her, along with SAMe, milk thistle, vitamins, and a lot of patience with the probable accidents the steroids may cause. On a lighter note, though she is really good at trying to make it to a hard floor if she can't make the door, I did invest in a carpet cleaner when we decided to rescue 2 more boxers, so......

Yesterday was a very difficult day for both of us. I hate feeling like there is nothing I can do to help her feel better when she is in obvious distress. Her breathing was extremely labored, she has the shakes, and she has fallen down several times. This morning she looked like Bambi all splayed out on the ice when she fell in the kitchen - she looked so sad and helpless :( She has absolutely no interest in eating right now either, though I did get her to eat a few tablespoons of oatmeal & quinoa this morning.

She seems to be resting a bit more comfortably today, but then again it seems to change on an hourly basis. Hopefully, the meds will kick in soon and start to relieve some of her symptoms. At least she is still willingly taking water and making urine.

Please keep her in your boxer prayers. grouphugicon

Very sorry to hear of your diagnosis. I hope your girl responds well to the meds. Don't hesitate to question the dosing for the pred. if she doesn't seem to be tolerating it well. Sometimes it isn't a one dose fits all type of drug. A lot of Boxers seem to be extra sensitive to a lot of different meds and lower doses that are most traditional can be utilized with the benefits that the normal dosing for other breeds require.

Thanks Susie. She is only taking 20mg a day plus a 225mg S-Adenosyl tablet & her soloxine of course. We will adjust if necessary once she ramps up and we see how she is responding.

She is looking a bit jaundice today, but she finally ate something. She hasn't been interested in food for 3 days, except for a small amount of oatmeal I was able to get her to eat yesterday. I got some canned Canidae chik&rice & mixed it with a scrambled egg, a little banana, & some lactose free cottage cheese & she decided it looked ok enough to at least try - she ate most of it, so I feel a little better. At least she is still drinking water & peeing, though it is still very dark yellow.

I'm so sorry for your loss. We lost our 5-year-old, Roxy, on September 3rd from a short 12-day illness from Lymphoma, and it all happened so fast that I still can't believe she's really gone. I'm still dealing with our loss so I can't give good advice as to when the pain stops, but I know it will in time. Wishing you and your family peace at this sad time.
