Dog Sitter Checklist???

Hello,
We are employing the help of some neighbor girls and their mom for the first 2 months or so after we get our new puppy. They will come into the house 2 times a day and take the puppy out, check water, etc. Does anyone have or know of a good checklist that I can print out and leave for the girls to fill out each day? I was hoping to find something that has boxes to check off if there were any messes to clean up, any toys chewed up, any water all drank, etc. If anyone knows of anything, please let me know. I think it would be more helpful than racing over to their house every day and asking them all these questions.

Gunther's sitter used to use a form, but now she just writes a short note on a note pad for me. She always tells me how many times he goes #1 and #2. And she will include any change in behavior, such as a day he drank an unusual amount of water, or when he is extremely energetic. She will write down if Gunther did a trick real well, or when he pulls out all his toy from his toy basket looking for "just the right one." I enjoy coming home to see what happened with Gunther that day and it alerts me to what to expect for the rest of the evening. As she is a professional pet sitter, the usual stuff like putting fresh water in his dish is routinely done.
